Every QR version adds 4 modules per side. Add the white border (quiet zone) on both sides to get the full bead count.

A cross stitch QR code is one full cross per dark module, usually in DMC 310 black on white Aida. Leave the light squares unstitched on white fabric, or stitch them too on colored fabric.
For machine embroidery, use the PNG as the artwork and keep at least 3 mm per module so the stitches don’t blur together.

Knit stitches are wider than they are tall, so a square QR chart knits up squashed and may not scan. Use duplicate stitch on stockinette, or add rows to correct for your gauge before you knit it.

Yes. A QR code is a grid of dark and light squares, so each square becomes one bead. Paste a link, download the free pattern and place black and white beads exactly as the chart shows — it scans once ironed.
A short link makes a version 2 code, 25 × 25 beads, or 29 × 29 with a 2-bead white border — exactly one large square Perler pegboard, about 14.5 cm across with midi beads.
Usually a single misplaced bead, too little white border, or glare from ironed plastic. Check the bead pattern against the preview, keep at least 2 rows of white around it, use higher error correction and scan in soft light.
Yes. Switch to cross stitch or knitting: each module becomes one stitch. On 14 count Aida a 29-square code is just over 2 inches wide; keep stitches dark and crisp so the phone can read the edges.
Medium is the best start. Low makes the smallest code; High makes a bigger code that still scans with a few mistakes or a small decoration on top.
